download and fill out a member Census

You may also use the online Employee Listing form to enter all your group members.

When completing the Member Census, make sure you do not alter the Field Titles and only enter the required information below each Field Title.

upload the completed census file

Complete the form below and upload the file unless you have already submitted the Group Employee List you created above.
